Health and Emergency Information
At the beginning of each year, parents and guardians are required to fill out and sign a Health Information Form. This form is part of the yearly online registration process and is available in Skyward Family Access all year long for parents/guardians to update their student's health information as needed. This form provides an annual update of your child's health and gives the school permission to authorize the transport and provide emergency treatment for your child if necessary.
It is important to remember to notify the nurse with any changes or concerns to your child's health and the school for any changes in the names and /or phone numbers of the emergency contacts for your child should they occur throughout the school year.
